Meghan Peters, our Quality Improvement Decision Support Specialist (QIDSS) and Anna Gibson-Olajos, Executive Director of Powassan and Area Family Health Team were awarded a poster of distinction at the recent 2016 Health Quality Transformation Conference for their work in monitoring performance with quality in mind. Their poster “Because You Care: Using your EMR Data to Save Lives” demonstrates the importance of using data to improve care.
By participating in AFHTO’s D2D data quality indicator, Powassan and Area Family Health Team (PAFFHT) were able to identify a serious issue in data quality for colorectal cancer screening. Through a number of change ideas including an intensive chart audit and implementation of best practices, PAAFHT was able to improve the data quality in this area to over 100%. The data in the EMR can have incredible implications and can make a difference in our patient’s lives.
Our QIDS Specialist, Meghan Peters, is an employee of the City of Lakes FHT and provides support to a partnership of 9 Family Health Teams across Northeastern Ontario. Many of these partners participate in AFHTO’s Data to Decisions (D2D) program which is a summary of the performance of primary care teams across Ontario on a small number of indicators that are both meaningful to providers and possible to measure. D2D is grounded in Ontario’s Primary Care Performance Measurement Framework (PCPMF) and reflects Barbara Starfield’s principles of comprehensive, patient-centered care.
