City of Lakes FHT team members from all four clinics gathered together on December 15th to discuss plans and improvements for 2018. The theme for the planning retreat was "Team Performance". 

The group brainstormed issues and ideas related to clinical services, clerical support, community partnerships and team culture. Much attention was given to improving both the patient and provider experience. Specific topics included discussions around decreasing wait times as well as emergency department and hospital admissions. Building strategic community partnerships and helping patients navigate the local healthcare system was also addressed. Team members articulated a shared vision for the next ten years including greater team collaboration, clinical expansion and leveraging emerging telecommunication technologies.
