Our Pharmacist provides pharmacy services to patients and providers of the CoLFHT.
This may include reviewing medications for patients; assisting with access to required medications (i.e. drug coverage); providing medication information and health teaching to patients; provide drug information to CoLFHT providers to improve medication use.
The Pharmacist is involved in the Hospital Discharge Program to help ensure appropriate medication use when patients are transitioning from hospital (or rehabilitation) to home. Patients/caregivers are contacted by the pharmacist within 7 days after hospital discharge. Medications are reconciled based on information in EMR, available hospital notes, and information from community and patient/caregiver. Drug related issues are communicated to appropriate providers (e.g. physician, community pharmacy, CCAC).
