Addressing Poverty in Primary Care

At the 2016  City of Lakes FHT Retreat, the Team identified a desire to pursue the development of a program that would assist patients who fall below the poverty line.


As a result we are participating is a pilot program coordinated by the Centre for Effective Practice, a not-for-profit organization and funded by the Local Poverty Reduction Fund.


The pilot project integrates a poverty screening tool into our EMR.


On September 8th, Dr. Samantha Green of St. Michael's hospital in Toronto and Dr. Amanda Hey, Primary Care Lead with the Northeast Cancer Centre, delivered a "Treating Poverty"education session offered by the Ontario College of Family Physicians.
