Quality Improvement Plan

Our Quality Improvement Plan (QIP) is focused on improving the patient and provider experience as well as clerical staff and our volunteer Board of Directors. We believe that by optimizing the experience of all team members, we are creating a high-performance team that will deliver an extraordinary level of primary care services to our patients. The goal of our QIP is to align our improvement efforts with the five strategic priorities of our teams’ 2022-25 Strategic Plan, as follows:

  1. Patient Centred Care will give our patients access to high quality primary care services by supporting patients during pandemic and post recovery, enhancing the patient experience, increasing patient engagement and embracing equity, diversity and inclusivity;
  2. Integrated Care will contribute to a better-connected health care system by collaborating with local partners, advocating for healthcare system transformation and making healthcare system navigation easier for patients;
  3. Digital Health will embrace digital health technologies by evolving our virtual care capabilities, optimizing the EMR system and adopting online health information tools for patients;
  4. Team Development will support the professional growth of all of our team members by enhancing the provider experience, recruiting & retaining a diverse and high-performance team, nurturing a healthy team culture and supporting research and learning opportunities; and,
  5. Organizational Sustainability will strengthen the long-term viability of the team by advancing succession plan strategies, protecting and expanding sources of revenue and effectively managing performance and risk.

Our IQ Team oversees our IQ Making Smart Improvements program. In recent years these IQ efforts have led to such improvements as a virtual care infrastructure, online appointment booking and a patient navigation program.

 

Our Quality Improvement Plan builds on the work of our previous QIPs. The affiliation between our FHT and physicians has been a highly collaborative relationship built on a mutual understanding of cooperative team work and quality improvement. Our physicians are members of a Family Health Network (FHN) and they have fully participated in the development and implementation of planning, programming and service delivery. Major inputs to our QIP planning process include our Board of Directors, IQ Team,  and our annual patient surveys, as well as HQO and AFHTO resources.
