At the City of Lakes Family Health Team, we know that health is influenced by much more than medical care alone. Accessing community services, financial supports, housing resources, and other programs can sometimes feel overwhelming.
Our Patient Navigator is here to help.
Through our Community Connects program, patients can receive individualized support to:
- Access community resources and services
- Connect with housing, food, and home care supports
- Navigate government benefits and applications
- Identify and address barriers to accessing services
- Receive compassionate guidance and emotional support
Appointments are available every other Wednesday and Friday from 9:00 a.m. to 12:00 p.m. Book your appointment through our website. Â
