Job Purpose

The City of Lakes FHT is recruiting a dynamic Quality Improvement Decision Support Specialist (QIDSS) to catalyze positive transformation across sixteen Family Health Teams in Northeastern Ontario. This role blends data analytics, change management, quality improvement, leadership, and digital health to enhance patient care delivery within Northeastern Ontario.

The QIDSS will work in close collaboration with the QIDSS Team for the Family Health Team partnership.  The current QIDSS team includes: Permanent QIDSS, two (2) temporary QIDSS, and a QIDSS Lead.  The QIDSS Lead will provide strategic guidance, set priorities, manage projects, and support decision-making to build capacity and ensure the success of the QIDSS role.

Family Health Team partnerships are located throughout: Manitoulin Island, Espanola, North Shores, Sault Ste Marie, Mattawa, West Nipissing, Powassan, Parry Sound, Sudbury, North Bay, Huntsville, Elliot Lake, Almaguin Highlands, and Iroquois Falls.

Key Duties & Responsibilities

 

 Patients Before Paperwork Initiative 

  • Support the development of an adoption and implementation plan for the onboarding and meaningful use of e-referral throughout FHT's across Northeastern Ontario
  • Monitor performance and usage of e-referral
  • Support through change management, training, and support as FHT's and providers onboard with e-referral
  • Support the development of any necessary policies and procesudres needed to onboard with e-referral
  • Collaborate and participate with local Ontario Health Teams in their e-referral initiatives regionally
  • Organize and engage in monthly meetings with e-referral partners including: Ontario Health, Local Delivery Group, and other QIDSS across Northeastern Ontario 

Data Management & Reporting

  • Collect, clean, and organize data from the EMR to support quality initiatives
  • Develop and maintain EMR tools to enhance data usability and workflow efficiency
  • Enhance data integrity, access, and comparability across FHTs
  • Develop and distribute user-friendly reports for performance monitoring and evaluation
  • Maintain communication with EMR providers and uphold strong documentation protocols
  • Complete regular reporting requirements for internal and external use including reports for Ontario Health

Coordination & Collaboration

  • Conduct site visits with the QIDSS Lead to assess and support local FHT needs
  • Support the QIDSS lead in conducting in-person evaluations of each FHT’s QI needs
  • Foster knowledge-sharing across teams and with provincial-level QIDSS colleagues
  • Attend quarterly partnership meetings and support the development of shared workplans
  • Champion the value of data-driven improvement and patient-centered care within the network
  • Work with the QIDSS Lead to expand capacity and scope across the partnership

Qualifications

  • Post-Secondary Degree
  • Experience in health administration and planning, health informatics, information management, business, or health care technology would be an asset 
  • Understanding of primary care and multidisciplinary team-based approaches to health care delivery;
  • Experience in health sector data management, working with electronic medical records and quality improvement initiatives;
  • Experience in developing and generating user-friendly reports;
  • Essential skills include problem solving, time and project management, interpersonal skills, analytical reasoning and communications;
  • Ability to manage multiple demands and interpret and evaluate data;
  • Demonstrated ability to provide individual and group training; and,
  • Ability to travel by car to remote locations.
