Join the City of Lakes Family Health Team (CoLFHT) as a Registered Practical Nurse and play a vital role in providing high-quality, patient-centered care while supporting innovative health programs. This dynamic position combines clinical nursing responsibilities with program coordination, making a meaningful impact on the health and well-being of our community.

The RPN reports directly to the CoLFHT Executive Director and works collaboratively with our interprofessional team to deliver comprehensive primary care and health programs.

RPN Duties

  • Provide direct primary care nursing services to patients of all ages
  • Conduct nursing assessments, triage, treatments, and health education in accordance with CNO standards
  • Maintain accurate and timely documentation; participate in chart reviews, case conferences, and referrals
  • Administer immunizations and report per Ontario Immunization Guidelines
  • Stay current with nursing best practices and pursue ongoing professional development
  • Deliver direct primary care to patients of all ages, including wellness checks, well-baby visits, and injections
  • Support chronic disease management, preventative screening, and health promotion
  • Collaborate with physicians, nurse practitioners, and allied health professionals
  • Opportunity to mentor and support interprofessional learners and students
  • Deliver care in clinic, community, and home settings
  • Lead infection control initiatives and report communicable diseases
  • Perform other duties as required to support evolving team needs
  • Support new patient intake, including new patient intake assessments, chart entry, etc.

 Program Support and Quality Improvement

  • Support the team in delivering programs and services to patients
  • Participate, support, and when appropriate lead quality improvement initiatives, programs and services
  • Supporting population health programs through digital patient communication
  • Assist with updating the website, and social media platforms with health promotion materials
  • Manage new patient waitlists, bookings, and digital communications
  • Organize program logistics including scheduling, setup, patient enrollment, and follow-up
  • Track and report program statistics; assist IHPs with administrative tasks
  • Support the immunization program by updating immunization records, contacting patients overdue for immunizations, and working with public health in vaccine ordering and ensuring vaccine stock is maintained and delivered to all 4 locations
  • Receive training, and provide training to staff related to Health and Safety including N95 Mask Fitting, and CPR training

Qualifications

  • Current registration as a Registered Practical Nurse with the College of Nurses of Ontario
  • CPR and First Aid certification
  • Minimum of two years experience
  • Effective listening skills, with ability to process information required to appropriately provide service, while recognizing boundaries and confidentiality
  • Strong communication skills and interpersonal skills
  • Experience with electronic medical records system is an asset
  • Bilingual an asset
